1. Click the button above. This will open a Google Sheet.
2. In the upper right, click File and Make a Copy.
3. Name the copy with your first and last name. Save it in a place that makes sense to you.
4. Once you have completed the form, download it as a PDF.
5. If you have already connected with a teacher, email them your
completed registration. Otherwise, email your completed
registration form to firstname.lastname@example.org.
